Two-compartment
A two-compartment pharmacokinetic model is a commonly used mathematical framework to describe how a drug distributes and is eliminated in the body after administration. In this model, the body is simplified into two interconnected spaces: a central compartment and a peripheral compartment. The central compartment typically represents the blood and highly perfused organs such as the heart, lungs, liver, and kidneys. The peripheral compartment represents tissues where the drug distributes more slowly, often including muscle and fat.After an intravenous bolus dose, the plasma concentration–time curve in a two-compartment model usually shows two distinct phases. The first phase is the distribution phase, during which the drug rapidly moves from the central compartment to the peripheral compartment. This phase is characterized by a steep decline in plasma concentration. The second phase is the elimination phase, during which the decline in drug concentration becomes slower and is primarily governed by elimination processes, such as metabolism and excretion, occurring mainly from the central compartment.Mathematically, the concentration–time profile in a two-compartment model is often described by a bi-exponential equation. The distribution and elimination phases are characterized by different rate constants, which can be estimated by fitting the model to experimental concentration–time data. Key pharmacokinetic parameters include the volumes of distribution for the central and peripheral compartments, the inter-compartmental clearance, and the systemic clearance from the central compartment. From these parameters, half-lives for the distribution and elimination phases can be calculated.The two-compartment model is especially useful when a drug shows rapid initial decline in plasma levels that cannot be explained by elimination alone. It allows for a more accurate description of situations where drug concentrations in tissues differ substantially from those in plasma for a period of time. This has practical implications for dosage regimen design, prediction of tissue exposure, and understanding the time course of both therapeutic and adverse effects.In clinical and research settings, the two-compartment model can be applied to various routes of administration, including intravenous infusion and extravascular dosing. When used correctly, it provides insight into the dynamics of drug movement within the body and helps support rational decision-making in drug development and therapy optimization.
