The four-compartment model is a conceptual and mathematical framework commonly used in pharmacokinetics, physiology, and biomedical engineering to describe the distribution and movement of substances within a system. In its most general form, the model divides the system into four distinct but interconnected compartments. Each compartment represents a region or functional space where the substance of interest is assumed to be uniformly distributed, and exchanges occur between compartments according to defined rate constants or transfer coefficients.In pharmacokinetics, a four-compartment model might represent the central compartment, usually associated with blood plasma, and three peripheral compartments corresponding to various tissues or organs. After administration, a drug first enters the central compartment, from which it can be distributed to the peripheral compartments and eventually eliminated. The rate at which the drug moves between compartments and is cleared from the system is typically described by a set of coupled differential equations. These equations incorporate parameters such as distribution rate constants, elimination rate constants, and volumes of distribution for each compartment.The central compartment often serves as the reference space for measuring concentrations, while the peripheral compartments represent tissues with different perfusion rates or binding characteristics. By distinguishing among these regions, the four-compartment model can capture complex distribution patterns that simpler models might fail to describe adequately.Beyond pharmacokinetics, four-compartment models are also applied in areas such as bone metabolism, cardiovascular physiology, and tracer kinetic analysis in medical imaging. For example, in bone studies, the compartments may correspond to blood, extracellular fluid, surface bone, and deep bone, with the model used to interpret the uptake and retention of labeled compounds. In imaging, a four-compartment model can separate free tracer, non-specifically bound tracer, specifically bound tracer, and metabolized products, allowing more accurate estimation of physiological parameters.One advantage of the four-compartment approach is its flexibility. By adjusting the structure of connections and the parameter values, the model can be tailored to different substances, species, and experimental conditions. It also provides a useful framework for fitting experimental data and for simulating how changes in physiological conditions or dosing regimens will affect concentration–time profiles. However, the increased number of compartments and parameters also introduces challenges, such as parameter identifiability and the risk of overfitting.
